Summary:
Within the context of a client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, the person promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.

Reporting to the Site & Resident Care Manager, Assisted Living Coordinator or designate, in alignment with the creation of a vibrant community, and working in collaboration with the interdisciplinary team, the LPN Supervisor is responsible for providing first line supervision to Assisted Living Workers and Activity Workers and providing nursing care to an assigned group of tenants according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN Supervisor coordinates the work of the Assisted Living Workers on the unit(s) including making and adjusting work assignments, providing work direction, scheduling staff, monitoring following up on the work of staff to ensure outcomes of care are achieved and ongoing evaluation of staff work. The LPN Supervisor functions as an independent practitioner, while recognizing situations such as increased tenant acuity or care that falls outside the scope of an LPN as appropriate for accessing the appropriate personnel for support and/or clinical intervention. Provides input into the development of individualized Personal Service Plans for tenants and ensures staff follows the Plans. Performs nursing care, including administration of medication and grooming/hygiene related duties as required by the Plan and documents as needed. Develops respectful and positive relationships with tenants, which are supportive of their choices. Ensures Coordinator/Site Resident Care Manager is kept appraised of the status of issues and concerns that have been raised and dealt with. Answers questions and inquiries from tenants, families and staff, and participates in a variety of meetings as needed. Maintains and orders supplies as needed.

Qualifications / Skills and Education:
Education
Grade 12, Graduation from a recognized program for Practical Nurses plus three (3) years’’ recent, related experience or an equivalent combination of education, training and experience. Registration with the BC College of Nursing Professionals as a practicing LPN registrant. Certificates in CPR and First Aid.

Duties and Responsibilities:
Supervises Assisted Living Workers and Activity Workers by making and coordinating work assignments, scheduling staff, sharing information and demonstrating tasks/clinical skills to ensure that Personal Service Plans are communicated, understood and followed. Troubleshoots staff issues and provides feedback to Coordinator as required/requested. Monitors and follows up on the work of staff to ensure outcomes are achieved. Conducts ongoing evaluation of staff work performance and provides related input to Coordinator/Resident Care Manager. Conducts regular medication and housekeeping audits as per established procedures.

Participates in the assessment process, development and modification of individualized Personal Service Plans that support the preferences and needs of tenants in an assisted living environment.

Provides nursing care as required such as administering medications, checking vital signs and blood glucose according to Personal Service Plan, and in alignment with the Assisted Living guidelines, agreements and philosophies.

Responsible for contacting Community partners or the Case Manager assigned to the tenant should there be a need for increased care, problems, unusual signs or symptoms or changes with the tenant.

Communicates with, provides support to, and encourages tenants to participate in social and recreational activities in the residence and activities of daily living such as personal care and grooming.

Models care philosophy, shares knowledge with staff and provides information and feedback to Coordinator/Site Resident Care Manager regarding developmental needs, safety concerns, orientation requirements, and other issues or concerns.

Identifies and recommends training/skill enhancements, participates in orienting new staff, and maintaining timekeeping and attendance records. Resolves staffing problems, including calling in staff to ensure appropriate staffing levels if required.

Completes and maintains documents and records related to tenants, their needs and clinical observations that may require documentation.

Orders medication refills for tenants if required, receives order and checks for accuracy. Stores medications in appropriate locations according to established protocols.

Identifies and responds to tenant and family learning needs, provides educational and reference materials, incorporates tenant and family teaching activities into care plan and nursing interventions, and/or makes referrals to other identified resources.
Acts as a resource by answering inquiries of tenant, family members and staff and takes follow up action. Relays issues and information to Coordinator as needed.

Participates in meetings such as Community Gathering, Care Conferences and other residence/staff meetings.

Provides input to the Coordinator regarding equipment and supplies being evaluated or required. Orders supplies according to established protocols if requested.

Provides support and assistance to Assisted Living Workers with light housekeeping, laundry and dietary duties for tenants as time permits.

Performs other related duties as assigned.
